When are winners announced?

What if I don't respond?

Who pays taxes on prizes?

What documents are required for the Grand Prize?
Winners of any prize valued at $600 or more will receive an IRS Form 1099-MISC and must provide a valid SSN or ITIN and a signed W-9 before the prize is delivered. 2. Eligibility+
Open to legal residents of the 50 U.S. states & D.C., 18+. Excludes Puerto Rico & other U.S. territories, recipients of Medicare/Medicaid/TRICARE or other government healthcare programs, K-DOC USA employees and their immediate families/household members.
3. Event period+
Begins May 7, 2026 at 6:00 p.m. PT and ends May 29, 2026 at 11:59 p.m. PT. All entry timestamps are determined by the Sponsor's system clock (Pacific Time).
4. How to enter+
Complete both: (1) free K-DOC Telehealth registration via the official link, and (2) submit your registration email on the official Google Form. One entry per person. Bots, multiple accounts, and automated entries are prohibited.
5. Prizes & ARV+
Grand Prize: 1 × Galaxy S26 Ultra (ARV $1,300). Second Prize: 5 × $50 Visa Gift Cards. Third Prize: 15 × $10 Amazon Gift Cards. Total ARV ≈ $1,700. Non-transferable, no cash equivalent. Sponsor may substitute a prize of equal or greater value.
6. Winner selection & notification+
Random drawing within ~7 days after Event Period ends. Notification by email/contact info provided. Winners must respond within 72 hours or forfeit; alternates may be selected.
7. Taxes+
All federal, state, and local taxes are the winner's responsibility. Prizes valued at $600+ require a signed W-9 and valid SSN or ITIN; failure to provide may result in disqualification. IRS Form 1099-MISC will be issued.
